Original Submission

The original submission to SpringerPlus was in the same general form as the final published version, as two separate articles. At the suggestion for the Executive Editor, the articles were combined into a single article before being sent out for external review.

A separate Detailed ethics statement was provided with the original submission. At the request for the Executive Editor, this was replaced by the Ethics Statement in the Methods section of the article.

Combined Article

Two external reviewers provided reports for the combined article; both recommended that it be separated into two separate articles.

Accepted Separate Articles

Part B was accepted for publication without further review because neither reviewer required any revisions to the content of that part of the combined article. Part A was accepted after the reviewers provided a final review report. See Data Files in sub-section menu at top of page for published files
